Informed Consent for Supervision & Recording of Counseling Sessions
This notice describes client rights, disclosures, and policies related to Rally Point Counseling Services. Please read it carefully. Rally Point may, at any time, add, change, or rescind any Policy or Procedure described in this document in its sole and absolute discretion, without notice.
This document acts in addition to all policy, client rights, or privacy information detailed in Rally Point’s “Counseling Policy & Client Rights” or “Notice of Privacy Practices” Documents. Feel free to discuss any item within this document with your counselor. Please read and indicate that you have reviewed this information and agree to it by filling in the checkbox at the end of this document.
General Information
At Rally Point, we believe that providing a great experience for you means that as Counselors, we learn and grow too. One way we do this is by meeting regularly as a clinical team to consult with each other about ongoing cases. This benefits you - because often you will have the experience of a team of counselors backing the therapeutic services you receive. In addition, Rally Point counselors are regularly supervised in sessions to ensure the highest quality of care. Below you will find details regarding the consent required for these practices within Rally Point.
Your participation is voluntary and confidential. Observation is for training and development purposes only. Observation is governed under HIPAA law and existing privacy practices.
Consent for Supervision
State laws governing mental health professionals and American Psychological Association Ethical Codes and the Association for Counselor Education and Supervision Ethical Standards require that you be informed that the work of your counselor is being supervised by a licensed or registered professional of the appropriate discipline.
Your counselor’s primary supervisor has full responsibility for the supervised work of their supervisee. In order to ensure the highest standard of care, supervisors monitor and review the progress of your work with your counselor. The limits of confidentiality delineated in our informed consent and privacy policy documents apply to this supervised practice. If you have any questions about this supervisory relationship, we encourage you to talk to your counselor. Supervisors at Rally Point are listed below. Signing this form acknowledges your informed consent for treatment by a counselor under supervision.

Consent for Recording and Review of Counseling Sessions
All Rally Point counselors have their counseling sessions recorded or observed live at times, so that supervisors and other clinical and administrative staff can monitor and oversee the quality of counseling. Recordings are secure and governed under HIPAA law and existing privacy practices. Your participation is voluntary and confidential.
It is also common for counselors under supervision to discuss their cases as part of their academic training. In this context, your counselor-in-training would not reveal your name.
Acknowledgment
Acknowledging this document means you
have read, understand, and agree to the above disclosures and policies;
have had a chance to ask questions about anything in this document that is unclear;
understand The purpose of recording and live observation is for training, supervision and consultation only as stated above;
understand any decision not to be recorded will not affect your eligibility for services, but may affect the timeliness of services;
understand you may request that the recorder be turned off at any time during sessions;
understand that you have the right to refuse to allow other staff to observe sessions at any time;
understand all recordings will be safeguarded appropriately under HIPAA & existing privacy standards;
understand you may discuss or clarify these issues with your counselor at any time;
give consent voluntarily;
give consent to Rally Point Clinical & Administrative staff to supervise clinical and administrative work conducted as a part of counseling services;
give consent for recording, and live observation, of your counseling sessions and for these recordings to be reviewed by clinical staff members/interns of Rally Point Counseling;
understand that this consent remains valid unless you or a legal representative withdraw your consent or are discharged from services.
Effective Date Of This Document
This document and the terms within went into effect on July 1st, 2024
